All the required forms to register and participate for the 2008 rides are now available.
All our merchandise forms are also available below.
You can download, print or view any of Hoagy's Heroes forms to your hard drive from this page. You must have MS WORD or Acrobat, Adobe Reader, or Acrobat Reader installed.
To download and save the form you want: 1. Right-click the link for the form, and then choose "Save Target As" from the pop-up menu. 2. In the "Save As" dialog box, select a location on your hard drive, and then click "Save".
To view or print the form you want: 1. Left click the link for the form. 2. If you selected the "pdf" version the form will be opened in Adobe, Adobe Reader or Acrobat Reader depending on which application you have installed. (Adobe viewer software is free but requires downloading the first time). You must click on "BACK" arrow to return to this page from Adobe. 3. If you selected the "W" or MS WORD version, a window will open asking you to "Open, Save or Cancel". You must have MS Word to open.
